How to Apply |

| 1. Call the national toll-free number* and leave your name, mailing address and phone number including area code. You will receive an application in the mail.
2. Read the application carefully before, filling it out. Use a blue or black ballpoint pen. Follow the written instructions; do not forget to sign and date the application. 3. Make an appointment with a counselor, domestic violence advocate, social worker or therapist. All applicants must have at-least one interview to connect them with local resources and confirms that their injuries were received from domestic violence and that the applicant has been out of the abusive relationship for at least a year. 4.Bring the application with you to the counseling appointment; tell about the domestic violence you've experienced; and have the counselor complete the form, sign it and attach a business card. 5. Make a copy of the application for your own files in case it gets lost. 6. Mail (DO NOT FAX) the original application to: NCADV, PO Box 18749, Denver, CO 80218-0749. 7. If there is a problem with your application, it will be sent back ro you. Once it is accepted, you will be notified in writing that your application was received and forwarded to the AAFPRS. 8. You will be contacted by the AAFPRS by phone with a referral to a doctor as close to you as possible. 9. Once an appointment is made, please notify the doctor's office if you must cancel. Not showing up without calling could jeopardize, your standing in the program. 10. During the consultation, the doctor will let you know what they can and cannot do for you, or whether you even qualify for the program. Since their services are free, the doctors have the final say on whether or not they can do the work that you need. There is no guarantee that you can be helped until you see the doctor for the first consultation. |
