Children's
Medical Services Program assists families in planning and obtaining specialty medical and
rehabilitative care for children who have or are at risk for chronic illnesses or
disabling conditions which interfere with normal growth and development. To qualify for
services, applicants must:

| Benefit | Covered services include hospitalization, outpatient specialty services, rehabilitative and habilitative services, durable medical equipment, disposable medical supplies, medications, nutritional supplements, and eligible dental services. | |
| Apply At | Local Health Department hospital social worker | |
| Appeals | You may request an informal conference if you disagree with the decision, and you may request a formal hearing. The time limits and how to appeal will be on the application form and on any adverse notices you get. | |
